Work Authorization 

Customer hereby authorizes OCTOPLUS MARINE SERVICE to do the repair work along with the necessary materials. OctoPlus Marine and its employees may operate the unit for purpose of testing, inspection or delivery at owner’s risk. An express mechanic’s lien is acknowledged on labor and materials to secure the amount of repairs thereto. 

It is understood that OctoPlus Marine will not be held responsible for any loss or damages to the unit (or articles left in or with the unit) in case of fire, theft, accident, inclement weather conditions or any other cause beyond OctoPlus Marine’s control. 

Customer further warrants that vessel is insured. 

Customer further acknowledges they are responsible for any non-mechanical aspects of the vessel.

It is understood that by reusing any used parts, that although OctoPlus Marine Service inspects and recommends any parts that are vital to completion, they are under no warranty and OctoPlus Marine Service will not be held responsible for failure of above stated parts. 

It is acknowledged and understood that upon receipt of invoice, the job is considered finalized, and any other issues will be a new work order. 

FINANCIAL POLICY 

It is also understood that no work will begin nor any schedule for work to begin will be made until OCTOPLUS MARINE SERVICE receives this authorization. A major credit card number and the expiration date of such, will be required.

Labor rate is $110.00 per hour, when deemed necessary and more time/cost efficient, a labor with helper rate will be applied at $60 more per hour per helper. After hour service (after 5pm, weekends and holidays) will be charged at $150 per hour. Each job is charged from our shop to your job and return trip.. 3.5% of labor is added to each invoice for miscellaneous shop materials and supplies not scheduled. 

Texas State sales tax will be charged at prevailing rates. Credit for the sales tax will be issued only after OctoPlus Marine Service is provided with a signed tax form and valid tax number registered with the State of Texas. 

Payment for all work performed is the responsibility of the vessel owner. Payment is due upon receipt. Any invoice not paid within 15 days will be charged to the credit card number provided. Interest will accrue at the rate of 1.5% per month on any invoices still unpaid after 15 days. If placed with an attorney for collection, you will be responsible for all attorney fees and court costs incurred in the collection process.  

Acknowledgement and Agreement of Policies 

By submitting a form via this website, I agree I have read the above paragraph(s) and hereby authorize OCTOPLUS MARINE SERVICE to repair my vessel. I understand that any further recommended work will be electronically/ verbally submitted. I understand that by electronically signing this document, I agree to be financially responsible for any work performed. 

WE RESERVE THE RIGHT TO MODIFY THESE TERMS AND CONDITIONS AT ANY TIME BY MAKING CHANGES TO THIS ANNOUNMENT
